We are writing to request your cooperation with our HEDIS Provider Location Table update. Our contract as a health plan with Ohio Department of Medicaid (ODM) requires we participate in medical record review based on HEDIS standards. This requirement is assessed by reviewing member records with a primary care practitioner during the last year. These records will be reviewed during the HEDIS season. This project is intended to gather information on the best contact for obtaining records for your members.
